The Prince & Princess of Wales Hospice offers specialist palliative care for patients with advanced and progressive disease, irrespective of underlying diagnosis.
The services at the hospice are available to those individuals aged 18 years and over with complex problems associated with a life-limiting or life-shortening illness, and where a progressive decline is evident.
Referrals for patients aged 16 years and over will be considered on an individual basis as part of the hospice’s development of a transitional care model.

Multidisciplinary inpatient care (14 beds).
Community based palliative care services delivered by a multidisciplinary community team.
A family support service and outpatient services comprised of day services, outpatient clinics and specific programmes of care.
Referrals are accepted from hospital consultants, GPs, or health and social care professionals, with the consent of the doctor who is managing the care of the patient.
GPs and district nurses are encouraged to refer electronically via SCI Gateway by selecting “Palliative Care Services GG&C” on the referral destination and following the step-by-step prompts thereafter in the “palliative services” and “clinical data” tabs.

Advice regarding this is available during office hours from the clinical administration team on 0141 429 9823. Completed referral forms should be submitted whenever possible by secure email (eg @nhs.net, @any.scot.nhs.uk, etc) to the designated hospice email address GG-UHB.PPWHClinicalAdmin@nhs.net
Fax and postal referrals should be avoided to ensure that referrals are triaged as quickly as possible and patient sensitive information is shared in accordance with Caldicott guidelines. However, if essential, a completed Prince & Princess of Wales Hospice referral form should be faxed from a secure line to the hospice clinical fax number 0141 429 8406 and receipt of this checked during office hours on 0141 429 9823.
Postal referrals must be marked private and confidential for the attention of a designated clinician.
Urgent referrals can be made by telephone to 0141 429 9823 during office hours or 0141 420 6785 out of office hours providing the referrer has immediate access to the patient’s record.
These calls will be referred to medical staff for a decision and must be followed up with a completed referral form as soon as possible using the recommended routes.
